PatientPoint® today announced an agreement with Mercy Health Physicians in Cincinnati, Ohio to provide technology solutions and services to help close gaps in care, improve medication adherence for chronic disease and strengthen patient engagement, all of which will elevate the patient experience.

Additionally, PatientPoint’s patient engagement, data analysis and content services will help Mercy Health Physicians leverage its investment in the Epic technology platform to improve patient outcomes. "By integrating PatientPoint’s content, library of preventative electronic screenings, and patient outreach capabilities to enhance the current infrastructure at Mercy Health, Mercy Health Physicians will be able to take action on any identified gaps in care as measured by HEDIS (Healthcare Effectiveness Data and Information Set) and implement an efficient care plan," said Raj Toleti, president of coordinated care solutions and chief technology officer, PatientPoint.
The PatientPoint implementation will extend the Epic platform and roll out gaps in care measurement tools using features such as Epic’s MyChart and integrated reporting dashboards. This program will permit the Mercy Health Select accountable care organization (ACO) to optimize quality measures, improve transitions in care and reduce readmissions.
