LUMEDX HealthView CVIS Partners with Ally® Connected Care to Power for Discharged Patient Follow-up

LUMEDX Corporation, a leading provider of cloud-based cardiovascular information systems (CVIS), and Continuity Health Solutions, maker of Ally® Connected Care and other outpatient disease management software, announced today that they have partnered to offer healthcare providers an integrated, longitudinal record for heart failure populations across the inpatient and post-acute care environments. Leveraging Ally® Connected Care platform embedded into LUMEDX’s HealthView CVIS, patients can perform daily, personalized self-assessments consisting of biometrics, medication compliance, education and questions that are automatically transmitted to their electronic health record.

The new solution will be unveiled at the American Association of Heart Failure Nurses Annual Conference, LUMEDX Booth 106, in Los Angeles, June 26-28, 2014.

“We are extremely excited about this relationship with LUMEDX and the unique opportunity it offers to extend the use case of telemonitoring into the workspace of cardiology care. A seamless, up-to-date view of the patient condition is a powerful tool for cardiology care teams. Integrating Ally® with HealthView cloud applications will enable caregivers to determine the best course of action for optimal care delivery if and when they have a patient present to an Emergency Room,” said CEO of Continuity Health Solutions, Bryan Poteet. “Our customers’ goals are to prevent readmission across the big three disease types—COPD, Diabetes and Heart Failure—through proactive management but circumstances will arise where patients present to the facility, and when they do the care team needs the best information to make an informed decision about which care pathway to pursue.”

The HealthView-Ally® Connected Care alliance delivers:

  • A seamless, integrated inpatient and outpatient cardiology record that helps providers reduce heart failure readmissions
  • A flexible, scalable solution, allowing providers to tailor the technology to meet the unique requirements of their workflow and individual patients
  • The ability to integrate data from patient populations beyond Heart Failure such as Diabetes and Chronic Obstructive Pulmonary Disease (COPD)
  • Complete, up-to-date patient information enabling appropriate, fully informed treatment decisions in the event a patient presents in the E.D.; patients can be sent to clinic rather than admitted
  • Meaningful analytics to support improved clinical and business outcomes

“Today’s healthcare organizations are looking for creative ways to offset regulatory pressures for quality and readmissions, while keeping the patient engaged in the care cycle,” said LUMEDX Vice President of Strategic Products, Praveen Lobo. “We at LUMEDX have been working to deliver innovative solutions to help our clients manage these pressures.”

“Connecting the inpatient HealthView cardiology record to the Ally® Connected Care platform offers cardiac care teams a way to engage their patients in a convenient self-management regimen while also providing caregivers with more easily accessible, comprehensive patient information,” said Lobo. “We believe that this combination of patient self-management and more fully integrated patient information will help hospitals better manage readmissions. We chose Continuity Health Solutions as a partner in this endeavor because they offer flexibility in their business and technology approach to meet the dynamics of today’s healthcare environment.”

Ally® Connected Care offers patients the opportunity to take an active role in their care regimen through an FDA-approved home appliance, an easy-to-understand automated call, or a voice-guided, browser-dependent web portal, allowing providers who are closest to the patient to determine which method of participation yields the highest compliance. LUMEDX’s HealthView platform is a cloud-based CVIS delivering a full suite of cardiology workflow solutions, integration tools, registries, services, and analytics.

Even though it’s now Microservices Journal, long-time fans of SOA World Magazine can take comfort in the fact that the URL – soa.sys-con.com – remains unchanged. And that’s no mistake, as microservices are really nothing more than a new and improved take on the Service-Oriented Architecture (SOA) best practices we struggled to hammer out over the last decade. Skeptics, however, might say that this change is nothing more than an exercise in buzzword-hopping. SOA is passé, and now that people are ...
