PointClickCare Strengthens Interoperability in Senior Care with Industry’s First Fully Integrated Continuity of Care Document

Continuing in its efforts to enhance the communications between senior care providers, hospitals, and accountable care organizations (ACOs), PointClickCare, the leading provider of cloud-base software for senior care, today announced new capabilities to integrate continuity of care documents (CCD) and lab results into its EHR platform. This level of integration enables senior care providers to bolster interoperability with their partners and improve connections with their provider networks. Collecting the required documentation to properly communicate resident needs at the time of transfer is time consuming, error prone and often incomplete. This can leave the hospital without sufficient, accurate information to provide the right care and services.
