PointClickCare Strengthens Interoperability in Senior Care with Industry’s First Fully Integrated Continuity of Care Document

Continuing in its efforts to enhance the communications between senior care providers, hospitals, and accountable care organizations (ACOs), PointClickCare, the leading provider of cloud-base software for senior care, today announced new capabilities to integrate continuity of care documents (CCD) and lab results into its EHR platform. This level of integration enables senior care providers to bolster interoperability with their partners and improve connections with their provider networks. These capabilities further PointClickCare’s efforts to enhance the communication between senior care providers, hospitals and accountable care organizations (ACOs), driving better care.

Collecting the required documentation to properly communicate resident needs at the time of transfer is time consuming, error prone and often incomplete. This can leave the hospital without sufficient, accurate information to provide the right care and services. Offering the senior care industry's first fully EHR-integrated CCD, PointClickCare’s platform automates the next generation of health information delivery. The receiving facility receives consistent data, in a uniform format, improving communication of treatment and care needs during transition of care.

With PointClickCare's Integrated Lab Results™, senior care providers are now able to communicate directly and rapidly with ancillary diagnostic services and exchange resident information through the facility's EHR. When results begin to trend outside of normal ranges, they are automatically flagged for review, allowing the clinical staff to adjust care plans and deliver appropriate care.
