CarePort Health and Jackson Health System Extend Care Management Partnership and Launch a First-of-its-Kind Integration with the Health System EHR

CarePort Health, a leading provider of care management and post-acute outcomes solutions, today announced that longtime Care Management client Jackson Health System will continue its partnership and extend its agreement to utilize CarePort’s discharge planning and referral management platforms at six Miami-area hospitals, medical centers and long-term care facilities. Additionally, CarePort and Jackson Health System developed a first-of-its-kind integration solution between the hospital’s Cerner EHR and the CarePort Care Management platform. The solution improves end-user workflow and enhances efficiency while providing a more seamless experience for staff to deliver the right care at the right time. This partnership helps Jackson Health System further expand its already-impressive quality of care services for residents of Miami-Dade County.

The newly created integration solution allows users to launch directly into the referral creation workflow within CarePort via single sign-on (SSO) from the Cerner application while keeping patient data accessible throughout both systems. The integration allows users to view and identify specific clinical data to create a referral specific to the patient’s clinical needs.

“At Jackson Health System, we strive to provide our patients with the highest level of quality care possible. In order to achieve this goal, we applied a highly-innovative strategy by designing and building the integration between these two disparate and unconnected systems,” said Michael Garcia, SVP & CIO at Jackson Health System. “This integration has enabled us to improve the delivery of quality patient care while also enhancing clinician satisfaction through the use of this innovative technology.”

Another key element of this bidirectional integration was to ensure that the final location of the patient’s placement is sent back to Jackson Health System’s Cerner EHR, thereby completing the referral process and increasing the visibility for providers across the care continuum. Jackson Health System is the first CarePort client to establish this level of integration between a Cerner EHR system and CarePort’s discharge planning application.

“Partnering with Jackson Health System to further embed our care management tools into the EHR is another step in our efforts towards improving transitions of care,” said Lissy Hu, CEO and founder of CarePort. “We look forward to continuing to partner with innovative customers such as Jackson Health to streamline care management workflows and innovate to improve patient outcomes across the continuum.”

An integrated healthcare delivery system, Jackson Health System consists of its centerpiece, Jackson Memorial Hospital, Jackson South Medical Center, Jackson North Medical Center, Holtz Children’s Hospital, Jackson Rehabilitation Hospital, Jackson Behavioral Health Hospital; a network of UHealth Jackson Urgent Care centers; multiple primary care and specialty care centers; and two long-term care nursing facilities. The new Jackson West Medical Center campus is under development in the City of Doral and will feature adult and pediatric emergency rooms and outpatient services.

Jackson Health System is a nonprofit academic medical system offering world-class care to any person who walks through its doors. Governed by the Public Health Trust, a team of citizen volunteers acting on behalf of the Miami-Dade Board of County Commissioners, Jackson Health System ensures that all residents of Miami-Dade County receive a single high standard of care regardless of their ability to pay.

Modern software design has fundamentally changed how we manage applications, causing many to turn to containers as the new virtual machine for resource management. As container adoption grows beyond stateless applications to stateful workloads, the need for persistent storage is foundational - something customers routinely cite as a top pain point. In his session at @DevOpsSummit at 21st Cloud Expo, Bill Borsari, Head of Systems Engineering at Datera, explored how organizations can reap the bene...
Serverless Architecture is the new paradigm shift in cloud application development. It has potential to take the fundamental benefit of cloud platform leverage to another level. Kubernetes is an open source system for automating deployment, scaling, and management of containerized applications. Kubernetes was originally built by Google, leveraging years of experience with managing container workloads, and is now a Cloud Native Compute Foundation (CNCF) project. Kubernetes has been widely adopted by the community, supported on all major public and private cloud providers, and is gaining rapid adoption in enterprises. However, Kubernetes may seem intimidating and complex ...
DevOps has long focused on reinventing the SDLC (e.g. with CI/CD, ARA, pipeline automation etc.), while reinvention of IT Ops has lagged. However, new approaches like Site Reliability Engineering, Observability, Containerization, Operations Analytics, and ML/AI are driving a resurgence of IT Ops. In this session our expert panel will focus on how these new ideas are [putting the Ops back in DevOps orbringing modern IT Ops to DevOps].
As you know, enterprise IT conversation over the past year have often centered upon the open-source Kubernetes container orchestration system. In fact, Kubernetes has emerged as the key technology -- and even primary platform -- of cloud migrations for a wide variety of organizations. Kubernetes is critical to forward-looking enterprises that continue to push their IT infrastructures toward maximum functionality, scalability, and flexibility.

The use of containers by developers -- and now increasingly IT operators -- has grown from infatuation to deep and abiding love. But as with any long-term affair, the honeymoon soon leads to needing to live well together ... and maybe even getting some relationship help along the way. And so it goes with container orchestration and automation solutions, which are rapidly emerging as the means to maintain the bliss between rapid container adoption and broad container use among multiple cloud host...
Serverless applications increase developer productivity and time to market, by freeing engineers from spending time on infrastructure provisioning, configuration and management. Serverless also simplifies Operations and reduces cost - as the Kubernetes container infrastructure required to run these applications is automatically spun up and scaled precisely with the workload, to optimally handle all runtime requests.
